Who provides hospice care?

Whether care is needed at home, in an apartment or in a skilled nursing facility, our team brings comfort, dignity and compassion to every step of the end-of-life journey. Hospice care is holistic, addressing not only medical needs but also emotional, social and psychological well-being.
Your hospice care team may include:
- Hospice Medical Director – A board-certified physician who oversees the patient’s care plan.
- Admissions Nurses – Nurses who help patients and families begin hospice care with understanding and ease.
- Hospice Nurses – Registered nurses (RNs) and licensed practical nurses (LPNs) who manage symptoms and provide skilled care.
- Hospice Case Managers – Nurses who coordinate care and ensure the care plan is followed.
- Hospice Aides – Certified nursing assistants (CNAs) who help with personal care and daily tasks.
- Medical Social Workers – Professionals who offer emotional support and help with resources and planning.
- Chaplains – Trained spiritual care providers who offer guidance and comfort, regardless of faith background.
- Bereavement Coordinators – Specialists in grief support who help families before and after a loved one passes.
- Hospice Volunteers – Trained individuals who offer companionship and support.
Our care team also actively works with primary care physicians and any other doctors involved in an individual’s care to ensure continuity and coordination throughout the care journey.
